Endoperiodontal lesions: new evidence for predicting endoperiodontitis

ABSTRACT
Analysis of the long-term results of predicting endoperiodontitis in patients with generalized periodontitis indicated the need to correct previously published data.
The aim of the study was to to conduct a comparative analysis of the prognostic criteria of endoperiodontitis and substantiate their expediency of using in the clinical practice of a dentist.
Objects and methods. Clinical and radiological studies were carried out in 73 patients with generalized simple and complex periodontitis aged 18–65 years and older. All patients underwent determination and analysis of prognostic signs of the development of the disease by means of constructing a previously developed and new program for predicting endoperiodontitis. In the previously developed program “Forecast of endoperiodontitis” (2019), prognostic signs of the development of the disease were additionally included. When carrying out diagnostic measures for patients, the new program for the prognosis of endoperiodontitis has expanded a number of basic diagnostic criteria. Assessing the state of the periodontal pocket and taking into account the peculiarity of its developmental mechanism, it turned out to be advisable, in addition to determining the depth, to include the criteria for its width, as well as to determine the number of pathological periodontal pockets in the area of all surfaces of the examined tooth.
Results and discussion. Long-term results of a study of the clinical state of periodontal tissues after 24 months showed good treatment results in 97% of patients, which indicates the advisability of using a new program for the prognosis of endoperiodontitis with monitoring of its use every 2 months. It should be noted that preventive diagnostics and advanced prediction of risk factors make it possible to stabilize the pathological process in the periodontal tissues of patients.
Conclusion. The prognostic criteria for the development of endoperiodontitis in patients with periodontal diseases have been clarified and supplemented. A new program for the analysis of prognostic signs based on extended prognostic signs is proposed to substantiate the use of complex therapeutic and prophylactic measures. A new valid method has been developed for predicting the likelihood of endoperiodontitis in patients with periodontal diseases based on regression analysis of prognostic criteria, which makes it possible to select the necessary therapeutic and diagnostic measures and the frequency of follow-up with constant monitoring of prognostic criteria. This made it possible to visually inform the patient about the level of risk of developing periodontal diseases, to conduct constant monitoring of prognostic signs, which increased the effectiveness of treatment by 30% in comparison with the previously developed program.
