Complex treatment of dental patients using fixed structures of dental prostheses on implants after reduction of the alveolar ridge

ABSTRACT
The aim of the study. To evaluate the effectiveness of complex treatment of dental patients using fixed dental prosthesis structures on implants after alveolar ridge reduction.
Objects and methods. A clinical case is presented to illustrate the stages of surgical and prosthetic restoration of function and aesthetics in a patient using temporary fixed dental prostheses on screw-retained dental implants. The implants were placed following alveolar ridge resection of the maxilla using bone-supported surgical guides. Prosthetic structures were fabricated from acrylic resin on a titanium bar using a photographic protocol, intraoral scanning, and prototyping.
Results and discussion. Tooth extraction, alveolar ridge resection to eliminate the gummy smile, and dental implant placement were performed simultaneously. Surgeries on the upper and lower jaws were carried out in two stages per the patient’s preference: first on the upper jaw, followed by the lower jaw after a few days. Temporary screw-retained prostheses on multi-unit abutments with full functional loading were fabricated and installed within seven days after implantation, followed by occlusal adjustments.
Conclusion. Reduction of the alveolar ridge and dental implant placement using bone-supported surgical guides significantly reduces surgical time and increases the accuracy of procedures. The use of virtual planning, photographic protocols for transferring data to the dental laboratory, and surgical guides during the intervention establishes a clear, sequential, and complete treatment process. Comprehensive rehabilitation of patients using temporary fixed dental prostheses on screw-retained implants, implemented under the “All-on-X” concept with the application of bone-supported guides, demonstrates high predictability and effectiveness. This approach significantly improves functional and aesthetic outcomes, ensuring rapid quality-of-life restoration and long-term clinical results.
