Effectiveness of twin-block functional removable appliance in treating skeletal class II malocclusion during the phase of active skeletal growth

ABSTRACT
Clinical decisions regarding the use of various types of orthodontic appliances such as functional or mechanical removable appliances, fixed orthodontic appliances, extra oral traction, as well as the choice between treatments with or without tooth extraction and the necessity of orthognathic surgery, are based on determining the phase of skeletal growth of the patient.
The aim of the study was to assess clinical effectiveness of twin-block functional removable appliance in treating skeletal class II malocclusion during the period of active skeletal growth according to the stage of cervical vertebrae maturation.
Objects and methods. The present case report describes different methods of growth analysis, and how with the help of cervical vertebrae maturation analysis method it was possible to indicate the stage of skeletal maturation and identify the adequate intervention time to perform growth modification of the jaw in a patient diagnosed with skeletal class II division 1 malocclusion.
Results and discussion. Using the cervical vertebrae maturation method, our patient was evaluated at cervical vertebrae stage 2, which is a favorable stage for active orthodontic treatment. Twin-block functional removable appliance therapy was applied to modify growth of the lower jaw, treat dental and skeletal malocclusion without the necessities of surgical complex treatment.
Conclusion. The use of twin-block functional removable appliance during the period of active growth of the facial part of the skull detected during the cervical vertebrae maturation analysis allows to create optimal conditions for the normalization of the position of the lower jaw in patients with distal occlusion.
