New method for assessing the results of treatment of peri-implantitis in patients at the stage of follow-up

ABSTRACT
The search for objective indicators of bone metabolism reflecting the course of bone regeneration in case of damage, and the possibility of using them to control healing processes, as well as for timely diagnosis of the onset of complications, remains an urgent issue of modern medicine.
The aim of the study is to develop and scientifically substantiate a method for evaluating the results of complex treatment of peri-implantitis based on biochemical parameters of oral fluid by evaluating the dynamics of an individual clinical situation.
Objects and methods. Clinical studies included 119 patients who were divided into two groups: 1 group of patients (61) with osteointegrated implants (control), 2 group (58) with periimplantitis. Oral fluid (OF) was taken in patients for the purpose of biochemical examination of markers of bone metabolism: the activity level of acid phosphatase and alkaline phosphatase, which were determined before treatment, as well as on the 3rd, 7th, 14th and 30th days after treatment.
Results and discussion. A favorable result of the treatment of peri-implantitis occurs with a decrease in the level of acid phosphatase activity on the 14th day of observations in the range of values from 27.0 U/l to 5.1 U/l and a simultaneous increase in the level of alkaline phosphatase activity from 18.7 U/l to 28.2 U/L. An unfavorable result of peri-implantitis treatment is determined if, on the 14th day, the dynamic level of acid phosphatase activity has consistently high values, and the level of alkaline phosphatase remains unchanged.
Conclusion. The proposed method for evaluating the treatment of peri-implantitis based on the biochemical parameters of the oral fluid allows: 1) simultaneously evaluate the processes of destruction and regeneration of bone tissue; 2) determines the level of activity of alkaline phosphatase and the level of activity of acid phosphatase as informative biochemical markers of the development of inflammatory and destructive complications as early as possible (on the 30th day) after dental implantation surgery; 3) allows you to personalize the approach to solving further questions about dental rehabilitation for a specific patient.
