Treatment of patients with complete edentulism using digital registration of the constructive bite

ABSTRACT
The aim of the study. To develop a method for registering a constructive bite using digital technologies to obtain occlusal relationships of dental arches when treating patients with complete edentia using immediate loading of dental implants during temporary fixed prosthetics in accordance with the previously habitual occlusion during removable prosthetics.
Objects and methods. The study involved 16 patients seeking dental care at the Department of Orthopedic Dentistry and Orthodontics with a Pediatric Dentistry Course at the Institute of Advanced Training and Preparatory Studies (IAP and PKZ) of the UO Bashkir State Medical University with a diagnosis of complete edentia. They were treated using a complete digital protocol developed at the department for transferring the position of the dentition and the position of the habitual occlusion from a functional removable denture to a temporary fixed structure with immediate loading of dental implants. The study focused on the specifics of planning and implementing surgical and orthopedic treatment for patients with this condition.
Results and discussion. A key clinical objective in full-arch implant-supported prosthetic rehabilitation – especially when transitioning from removable dentures to fixed prostheses under immediate loading conditions – is the reliable and reproducible transfer of the dental arch position and habitual occlusion. The success of a fully digital protocol directly depends on the accuracy of intraoral scanning, the correctness of digital data registration/matching, and the stability of occlusal relationship transfer within the CAD/CAM environment. Digital occlusal transfer enables a 30–40 % reduction in treatment time and improves patient satisfaction.
Conclusion.  The method of registering the constructive bite using digital technologies to obtain occlusal relationships of the dental arches in the treatment of patients with complete edentia using immediate loading of dental implants during temporary fixed prosthetics in accordance with the previously habitual occlusion during removable prosthetics is a clinically valid and reproducible method that improves the functional and aesthetic results of treatment.
