Features of diagnosis and surgical treatment of patients with fractures of the naso-orbital-ethmoid complex of the face

ABSTRACT
Fractures of the naso-orbital-ethmoid complex (NOE) complex may be accompanied by or lead to serious consequences, including cerebrospinal fluid rhinorrhea as a result of violation of the integrity of the ethmoid bone in the base of the skull in a frontal-basilar
skull fracture, functional ophthalmic disorders due to deformation and displacement of the fragments of the medial wall of the orbit, medial canthal ligament disruption resulting in telecanthus, diplopia. Aesthetic disorders in NOE trauma are caused by a violation of
the position of the eyeballs, as a result of displacement of the medial walls of the orbits (traumatic orbital hypertelorism), a decrease in the orbital space, displacement of the fragments of the nasal bones and maxilla frontal processes that form the pyramid of the external nose, and deformation of the nasal septum. Presented the basic principles of diagnosis and choice of surgical access based on the results of literature analysis and own results in the surgical treatment of NOE fractures accompanied by other midface bone fractures/ The coronal access is preferable in the combination of NOE fractures with fractures of the frontal bone and the upper walls of the orbits. Reposition of the NOE complex through approaches in the periocular area (subciliary, subtarsal) with additional intraoral reposition and closed intranasal reposition allow to achieve satisfactory functional and aesthetic results.
