Effect of diabetes mellitus types 1 and 2 on the effectiveness of professional oral hygiene among patients with chronic generalized marginal periodontitis

ABSTRACT
The aim of the study. To check the effectiveness of various complexes of professional oral hygiene among patients with chronic generalized marginal periodontitis depending on the type of diabetes (D).
Objects and methods. The study included 20 patients with DM type 1 and 20 patients with DM type 2 aged 45 to 55 years, diagnosed with mild or moderate chronic generalized marginal periodontitis. Patients underwent determinationof hygienic and periodontal indices, measurement of blood flow velocity, pH-metry of the oral fluid, microbiological analysis of the contents of periodontal pockets, 2 different professional oral hygiene complexes were performed.
Results and discussion. After both professional oral hygiene complexes, a decrease in values of dental indices was abided in all groups of patients. After the professional oral hygiene complex with the additional use of the Vector device, growing the level of the acid-base state of the oral fluid was noted. Volume and linear blood flow rates increased by 0.09– 0.14 ml/min after professional oral hygiene 1 month after the procedure, however, there was a decrease in the rate after 6 months. The microbial composition of the contents of periodontal pockets showed the presence of a greater number of pathogenic species in DM type 2 and a more significant decrease in the number of pathogenic microorganisms after the professional oral hygiene complex with gentle ultrasonic treatment with the Vector device among patients with DM type 1.
Conclusion. The microflora of periodontal pocket of patients with DM type 1 is the subject to therapeutic effects in the form of professional oral hygiene, especially with the use of treatment of periodontal pockets with the Vector device, while the parameters of the periodontal microcirculatory bed are restored for a long period. The effectiveness of professional oral hygiene is better in the group of patients who has periodontitis in chronic form with DM type 1 than among patients with DM type 2.
