Biochemical parameters of oral fluid in patients with traumatic fracture of mandible in presence of third molars in oral cavity and without them

ABSTRACT
The aim of the study of the work is to analyze biochemical parameters (the level of activity of alkaline phosphatase (alkaline phosphatase) and the level of calcium ions Ca2+)) oral fluid (OF) in persons with traumatic fractures of the lower jaw in the presence of third molars in the oral cavity and without them and to determine the direction of the influence of them on homeostasis and mineral metabolism in the oral cavity.
Objects and methods. 82 persons aged 18-60 were examined. Group 1 consisted of 43 patients with mandibular fractures who had more than 2 third molars. Group 2 – 18 patients with mandibular fracture who did not have third molars. Group 3 (control) included 21 healthy persons who had more than 2 third molars. The groups had no differences in age, gender, somatic and dental status. The level of activity of alkaline phosphatase and the level of Ca2+ in oral fluid content were determined on a semi-automatic analyzer “Clima MC-15” with a standard set of reagents.
Results and discussion. Significant differences between group 1 and control using the Kraskel-Wallis criterion z = 3.372 (H = 12.218, p = 0.002) in the level of activity of the alkaline phosphatase and the level of Ca2+ ions z = 2.672 (H = 8.767, p = 0.013) were revealed.
Conclusion. Jaw injury causes tissue ischemia, promotes activation of anaerobes creating favorable conditions for their development which is important when there are 2 or more third molars in the oral cavity which, due to pathological processes associated with these teeth, disrupt oral homeostasis and cause an increase in the level of endogenous intoxication which convinces of the need to use a differentiated approach more widely to the preservation of the third molars in the oral cavity.
