Prediction of progression of generalized periodontitis in patients with rheumatoid arthritis

ABSTRACT
Predicting the progression of generalized periodontitis is paramount for determining treatment and monitoring tactics, which can significantly improve the results of periodontitis treatment.
The aim of the study. To study the signs of generalized periodontitis and determine its prognostic criteria for the course of patients with rheumatoid arthritis (RA) based on clinical and laboratory data.
Objects and methods. . 66 patients with seropositive RA were examined, including a comprehensive assessment of the periodontal condition with the calculation of generally accepted indices and radiological diagnostic methods, determination of the level of antibodies to cyclic citrullinated peptide, rheumatoid factor and RA activity index DAS-28.
Results and discussion. Generalized periodontitis was identified in 32 people. The main criteria for assessing the severity of generalized periodontitis in patients with RA were established: the value of the periodontal index, the number of segments with bleeding during probing in relation to the total number of segments, as well as the number of segments with a pathological periodontal pocket depth of 5 mm or more.
Conclusion. A method has been developed for determining the probability of generalized periodontitis progression in patients with RA, which includes the assessment of 8 prognostic parameters assessed on a sixgrade scale. The probability of possible progression of generalized periodontitis was assessed as low, medium and high depending on the area of the prediction chart in percentage with corresponding color marking in green, yellow and red, which made it possible to visually and quantitatively assess the risk of progression of generalized periodontitis.
