Prosthetic stomatitis: assessment of the effectiveness of diagnosis and treatment

ABSTRACT
One of the frequent complications in the use of removable acrylic dentures is denture stomatitis. At present there is no any method of estimation of severity and efficiency of treatment of denture stomatitis, which would allow to estimate the severity of inflammatory process of denture bed tissues and the degree of its elimination in numerical expression.
The aim of the study. To develop and approve in the clinical practice of prosthetic dentistry the method of estimation of severity and efficiency of denture stomatitis elimination.
Objects and methods. Complaints and clinical data from 76 patients aged from 36 to 72 years who used different designs of removable acrylic dentures were studied to identify the most frequent complaints and symptoms encountered in denture stomatitis. Subsequently, based on their analysis and the dynamics of clinical changes in the course of denture stomatitis treatment by different dentists, a method of assessing the severity of denture stomatitis and the effectiveness of its treatment was developed, which was tested in the dynamic observation of 19 elderly patients who used full removable dentures for a year and had denture stomatitis.
Results and discussion. The technique of assessment of severity of the course and effectiveness of treatment of denture stomatitis on the basis of analysis of complaints and clinical symptoms, namely: pathological sensations of the patient, color of the oral mucosa in the area of the denture bed and field, presence of traumatic lesions on the oral mucosa, moisture of the oral mucosa of the tongue, as well as taste disorders was developed. Approbation of the technique showed that it allows differentiating the severity of the course of prosthetic stomatitis. In the dynamics of the patients’ observation the efficiency of prosthetic stomatitis therapy according to the proposed method amounted to 93,18 % on the 5th day from the moment of the initial examination of the patients, which speaks about the rational work of the dentist and effective conservative treatment of prosthetic stomatitis with the use of means for improving the fixation of the prosthesis.
Conclusion. The developed technique of estimation of the severity of the course of denture stomatitis is easy to use, does not take much time in the work of a dentist and can be used in clinical practice, including for comparison of the effectiveness of treatment of denture stomatitis with various drugs.
