Changes of biochemical indices of oral fluid in operation of the prosthetic with support on dental implants

ABSTRACT
The aim of the work is to study the changes in the biochemical indices of oral fluid (OF) in the functioning of orthopedic structures based on dental implants.
Objects and methods. We examined 57 patients 25–52 years old. Group 1 − 42 patients who had surgical dental implantation and are preparing to fix the fixed orthopedic structures based on dental implants. The control group included 15 patients who were examined at the same time, dental implantation was not performed. In dynamics, the level of activity of acid phosphatase (ASP) and alkaline phosphatase (ACP), the level of calcium ions (Ca2+) was determined: 6 months after surgery and before orthopedic treatment, 1 day after fixation of the orthopedic structure, 3, 14, 21 days and 6 months.
Results and discussion. In patients of group 1 the day after fixation of orthopedic structures was observed increase in the studied parameters of OF (p < 0.05). On the 3rd day the duality of their dynamics was revealed, which allowed to distinguish two subgroups. In 36 (86%) patients of subgroup A, the values of the investigated levels of activity of CF and GF and the level of Ca2+ ions decreased (p < 0.05). At the same time in 6 (14%) patients of the subgroup B indices were not changed (p > 0.05). In further research the indices in sub-group continued positive trend and in the subgroup maintained a high value. This was confirmed by the data of the clinic. 100% of patients of subgroup B were diagnosed with peri-implantitis on 21 day. In 4 (9.5%) patients, dental implants were removed, 2 (4.8%) after complex treatment continue to function.
Conclusion. The results confirm that fundamentally new data are objectively based on the principles of evidence-based medicine confirming the presence of the second critical period in dental implantation, which begins immediately after the fixation of orthopedic structures on a permanent basis and the beginning of their active use by the patient in the process of life which must necessarily be taken into account in the complex treatment and preventive measures accompanying.
