Systemic inflammatory response syndrome in patients with odontogenic phlegmons the maxillofacial region

ABSTRACT
The aim of the study is to determine the features of the development of systemic inflammatory response syndrome (SIRS) in patients with odontogenic phlegmons of the maxillofacial region.
Objects and methods. Surveyed 158 patients with infectious-inflammatory process of maxillofacial area and neck, which were divided into 3 groups: 1 group (96 people) – with acute purulent odontogenic osteomyelitis of mandible complicated by the phlegmon cellular spaces to one space, group 2 (36 patients) with acute purulent odontogenic osteomyelitis of mandible complicated by the phlegmon of several (2–4) cellular spaces, group 3 (26 people) – with acute purulent odontogenic osteomyelitis of the mandible complicated by the phlegmon of bottom of cavity of mouth. Laboratory tests of patients’ blood (general and biochemical analyses) were performed. Based on the general analysis of blood was carried out calculation of integral indices of intoxication. Determination of interleukin-8 and procalcitonin levels was performed using an enzyme immunoassay. The level of activity of blood elastase and oral fluid was studied.
Results and discussion. Acute odontogenic osteomyelitis, complicated by phlegmon of the adjacent soft tissues, is characterized by the development of SIRS. With phlegmons of one cellular space it develops in 9.0% of patients, with phlegmons of 2–4 cellular spaces – in 36.0%, with phlegmons of the bottom of the oral cavity – in 80.0%.
Conclusion. The development of odontogenic infectious diseases of the maxillofacial region is accompanied by SIRS. To prevent the development of severe complications, it is necessary to take into account the localization, prevalence of the process, as well as the appearance of laboratory markers of impaired functioning of macroorganism systems. The most promising indicators for this purpose should be considered the level of albumin, procalcitonin, interleukin-8 and serum elastase activity, which requires further directed research.
