Immediate implantation and immediate loading of dental implants with non-removable screw-retained prostheses manufactured using additive and CAD/CAM technologies

ABSTRACT
The aim of the study. To demonstrate the possibility of comprehensive functional and aesthetic rehabilitation of a dental patient using additive and CAD/CAM technologies in the manufacture of temporary and permanent designs of dentures.
Objects and methods. The method of rehabilitation of fully edentulous patient by non-removable implant-supported screw-retained monolithic zirconia structures manufactured utilising CAD/CAM process is described. During the treatment, the Carestream CS3500 intraoral scanner and AlphaBio Tec scan markers with the connection interface corresponding to the installed implants were used to obtain optical impressions. Temporary prostheses were designed in Exocad CAD software and were manufactured from the biocompatible polymer “Denture Teeth A2” using additive technology using a Form 2 printer (Formlabs). Final prostheses were milled out of multilayer high translucent zirconium dioxide blocks, sintered and glazed.
Results and discussion. Teeth extraction and dental implantation under local anaesthesia was performed in one step. Dental implants were inserted into the toothless areas of the alveolar processes and into the alveolae of the extracted teeth in two stages, related to the patient’s wishes: first on the maxilla, then on the mandible. Within two weeks after implant placement, temporary screw-retained implantsupported prosthetic structures were delivered and functionally loaded. Temporary dentures were replaced with final restorations in 2.5 months. The treatment period was 4 months, the long-term follow-up period from treatment completion was 2 years.
Conclusion. When possible, the use of completely digital protocols for prosthetic rehabilitation of partially or fully edentulous patients using non-removable implant-supported screw-retained monolithic zirconia structures demonstrates their high efficiency.
